As a Nurse Practitioner specializing in home health, you will play a vital role in assessing and educating Medicare Advantage plan members, making a meaningful difference in their well-being through comprehensive, in-home assessments.

Schedule:

• Flexible shifts available seven days a week
• Ability to create a personalized schedule
• Choose your minimum weekly or bi-weekly commitment

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Gather and review patient medical history, current conditions, and medications
• Educate patients on their medical conditions and proper medication management
• Identify care gaps and provide actionable recommendations to improve health outcomes
• Conduct visits in Woodlawn, Cunningham, Sango, and other Montgomery County locations
• Focus solely on assessmentno treatment planning or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• 6 months of recent experience as a Nurse Practitioner is preferred
• Graduate of an accredited Master of Science in Nursing (MSN) program
