Job Description
Make an impact in healthcare by conducting comprehensive home health risk assessments for Medicare Advantage plan members across Montgomery County. This per diem role offers exceptional autonomy and flexibility, allowing you to design your own schedule with shifts available any day of the week, Monday through Sunday. Assignments may include home visits in Woodlawn, Cunningham, Sango, and the surrounding communities within Montgomery County.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment
Core Responsibilities:
- Perform annual and as-needed home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients current and prior medical history, ongoing medical conditions, and medication use
- Offer education to patients on managing medical conditions and understanding prescribed medications
- Identify gaps in care and provide actionable recommendations to help close those gaps
- Role excludes prescribing medications or creating treatment plansyour focus is on comprehensive assessment and patient education
Requirements:
- Active Nurse Practitioner license in Tennessee
- Current BLS certification (CPR)
- Board Certified through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- A minimum of six months recent Nurse Practitioner experience preferred
