Deliver high-quality healthcare in a home setting as a Nurse Practitioner specializing in home health risk assessments. In this per diem position, you will provide compassionate care to Medicare Advantage plan members throughout Craig County, including Vinita, Ketchum, Welch, and White Oak. Each assessment typically takes 45 minutes and may include annual health reviews or post-discharge follow-ups, supporting patients ongoing health and wellness.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your personal and professional needs
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive home health risk assessments on Medicare Advantage members
- Review and evaluate prior and current medical history, chronic conditions, and medication lists
- Educate patients about their health conditions and medications to ensure optimal outcomes
- Identify gaps in care and recommend appropriate next steps to support wellness
- No requirement for treatment planning or medication prescribing
Qualifications:
- Active Nurse Practitioner License in Oklahoma
- Current CPR certification (BLS required)
- Certified through either the AANP or ANCC
- Preferably at least 6 months of experience working as a Nurse Practitioner
- Master of Science in Nursing (MSN) from an accredited program
