Job Description

Take on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health services. In this role, youll conduct thorough home health risk assessments for Medicare Advantage plan members throughout Craig County, OKincluding Vinita, Ketchum, Welch, and White Oak. Each assessment averages 45 minutes and may involve annual health assessments or post-discharge visits, allowing for impactful, patient-centered care in the comfort of individuals homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule to best fit your lifestyle
  • Choose minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review and evaluate patients prior and current medical histories, existing conditions, and medication regimens
  • Educate patients regarding their health conditions and prescribed medications
  • Identify care gaps and offer actionable recommendations for optimal patient outcomes
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Oklahoma
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of six months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program
