Make a meaningful impact in patient care by performing comprehensive home health risk assessments for Medicare Advantage plan members in Pontotoc County, OK. As a Nurse Practitioner specializing in home health, you will visit patients in their residences within Ada, Allen, Stonewall, and nearby areas, providing essential healthcare insights without the responsibility of ongoing treatment plans or prescribing.

Key Responsibilities:

• Conduct detailed annual health risk assessments in patients homes
• Review patients' prior and current medical histories, conditions, and medication regimens
• Offer clear education and support regarding medications and chronic health conditions
• Identify potential care gaps and provide thoughtful recommendations to enhance patient outcomes
• Each assessment typically requires about 45 minutes per visit; assessments may include annual wellness checks or post-discharge evaluations

Qualifications:

• Active Nurse Practitioner license in the state of Oklahoma
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of current experience as a Nurse Practitioner is preferred

This per diem position features pay of $80$90 per evaluation, plus a mileage stipend and compensation for administrative tasks.
