Job Description

Deliver expert care and make a positive impact on the lives of Medicare Advantage members by performing comprehensive home health risk assessments. This per diem opportunity is perfect for skilled Nurse Practitioners seeking flexibility, autonomy, and the chance to make a real difference in patients well-being. The role centers around conducting visits throughout Harford County, assessing individuals in their homesevaluating everything from annual wellness needs to post-discharge transitions.

Schedule:

  • Flexible shifts available seven days a week
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform thorough home health risk assessments for Medicare Advantage members
  • Review and evaluate prior/current medical history, medical conditions, and medication records
  • Educate patients regarding conditions, medications, and overall health management
  • Identify and document care gaps, sharing actionable recommendations
  • Focus exclusively on assessmentno treatment planning or medication prescribing

Preferred Qualifications:

  • Active Nurse Practitioner licensure in Maryland
  • Board certification via AANP or ANCC
  • Current CPR certification (BLS required)
  • Completed MSN degree from an accredited nursing program
  • At least 6 months of recent NP clinical experience (strongly preferred)
