- Excellent clinical judgment and strong documentation abilities
- Dependable transportation for conducting home visits
- Ability to work independently and efficiently manage your caseload
Primary Responsibilities:
- Deliver skilled, individualized nursing care to home health patients following established care plans
- Assess and monitor patient progress, updating care plans to reflect changes in condition
- Administer medications and carry out treatments according to state regulations
- Educate patients and their families on self-care techniques and condition management
- Maintain thorough, accurate records via HomeCare HomeBase (HCHB) EMR
- Collaborate effectively with registered nurses, therapists, and other healthcare team members to ensure comprehensive patient care
