Job Description
Experienced and compassionate Licensed Practical Nurses are invited to provide high-quality nursing care to patients in their homes. This contract opportunity is best suited for professionals who thrive in a field-based environment, excel at providing patient-centered care, and value independence and flexibility.
Schedule:
- MondayFriday, 8:00 AM 5:00 PM
- Occasional flexibility based on patient needs
Responsibilities:
- Conduct skilled nursing visits and create individualized care plans for home health patients
- Administer medications, perform wound care, and manage IV therapy or other clinical interventions as ordered
- Monitor patient conditions and communicate changes to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medication, and self-care techniques
- Document all assessments, interventions, and communications using Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary team to coordinate and ensure compliance with care plans
- Maintain infection control and patient safety at all times
- Attend case conferences, participate in quality improvement activities, and complete required training
Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or a similar community-based care setting
- Proficient with Home Care Home Base (HCHB) software
- Valid drivers license, reliable transportation, and current auto insurance
- Current Basic Life Support (BLS) certification
- Excellent clinical judgment, time management, and interpersonal communication skills
- Ability to work independently and manage a dynamic caseload
Preferred Experience:
- Previous work with Medicare-certified home health agencies
- Knowledge of home health compliance and documentation best practices
