Job Description
Deliver compassionate, patient-focused care as an experienced Home Health Licensed Practical Nurse (LPN). In this contract position, you'll support individuals in their homes, fostering independence, well-being, and recovery. You'll manage a caseload within a designated territory, using your clinical expertise and a strong commitment to quality to shape each patient's journey.
Schedule & Work Details:
- Monday Friday, 8:00 AM 5:00 PM (some flexibility based on patient needs)
- Workload: 2532 points weekly (equivalent to roughly 35 patient visits per day)
- Field-based: All care delivered within patients homes
- Charting system: Home Care Home Base (HCHB) proficiency is required
- Reliable transportation for travel between visits
Role Responsibilities:
- Perform skilled nursing assessments and develop individualized care plans
- Administer medications, wound care, IV therapy, and other treatments
- Monitor and report changes in patient condition to RN Case Manager or physician
- Educate patients and caregivers on disease management, medication, and self-care
- Ensure timely, accurate documentation using Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary team to ensure regulatory and clinical care standards
- Practice infection control and safety measures diligently
- Participate in case conferences and ongoing professional education
Qualifications & Experience:
- Active, unrestricted LPN license in the state of practice
- At least one year of experience in home health, hospice, or related settings
- Proficiency in Home Care Home Base (HCHB) software
- Current drivers license, reliable vehicle, and insurance
- BLS (Basic Life Support) certification
- Strong time management, critical thinking, and communication skills
- Ability to manage a dynamic, independent caseload
Preferred:
- Experience with Medicare-certified home health agencies
- In-depth understanding of home health compliance and documentation
