Job Description

Deliver high-quality, patient-centered care as a field-based Home Health Licensed Practical Nurse (LPN). In this essential role, you will support adult patients in their homes, fostering greater independence, supporting recovery, and providing critical education to patients and families. Youll coordinate closely with interdisciplinary team members and ensure the highest standards of compliance and documentation.

Schedule & Details:

  • Schedule: Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
  • Workload: 2532 points per week (approximately 35 patient visits daily)
  • Setting: Field-based, serving patients within your assigned territory
  • Charting System: Proficiency in Home Care Home Base (HCHB) required
  • Travel: Must have reliable transportation

Key Qualifications:

  • Active, unrestricted LPN license in Oregon
  • At least 1 year of experience in a home health, hospice, or community care setting
  • Prior use of Home Care Home Base (HCHB) electronic charting system
  • Valid drivers license, reliable vehicle, and current insurance
  • Current BLS certification
  • Excellent clinical assessment, time management, and communication skills
  • Ability to work independently in the field and manage caseload efficiently

Preferred Experience:

  • Background with Medicare-certified home health agencies
  • Thorough understanding of home health compliance and documentation standards
  • Compassionate, empathetic patient care approach

Core Responsibilities:

  • Conduct skilled nursing visits and manage individualized care plans
  • Administer medications, wound care, IV therapy, and clinical interventions as prescribed
  • Monitor and report changes in patient status
  • Provide patient and caregiver education on disease, medication, and self-care
  • Collaborate with the care team on plan of care coordination
  • Uphold infection control, patient safety, and quality standards
