Job Description
Provide compassionate, patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN) delivering skilled services in patients homes within the Woodburn, OR area. This full-time contract position focuses on supporting patient independence, recovery, and education, while maintaining clear communication with patients, families, and the healthcare team.
Responsibilities:
- Conduct skilled nursing visits to assess patient conditions and develop individualized care plans
- Administer medications, perform wound care, and manage IV therapy or other clinical treatments as prescribed
- Monitor patient status changes and promptly report to the RN Case Manager or physician
- Educate patients and caregivers on disease management and self-care techniques
- Document assessments, treatments, and communications accurately using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams to coordinate comprehensive patient care
- Ensure infection control and patient safety at all times
- Participate in case conferences, quality improvement initiatives, and required trainings
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of experience in home health, hospice, or community-based nursing
- Proficiency with Home Care Home Base (HCHB) documentation software
- Valid drivers license, reliable vehicle, and current insurance for travel within assigned territory
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, organizational skills, and effective communication abilities
- Ability to independently manage a caseload in a field-based role
Preferred:
- Experience with Medicare-certified home health agencies
- Knowledge of home health compliance and documentation standards
- Patient-centered approach with excellent interpersonal skills
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility to meet patient needs
- Approximately 35 patient visits per day (2532 points weekly)
