Job Description

Step into a vital healthcare role supporting patients recovery and independence in a home setting. In this contract position, youll serve as a Home Health Licensed Practical Nurse (LPN), delivering patient-centered nursing care and ensuring a positive impact on individuals lives. Your expertise in home health, clinical interventions, and documentation will be key as you guide patients through their care journeys.

Schedule:

  • Monday through Friday, 8:00 am to 5:00 pm
  • Occasional flexibility for patient needs

Responsibilities:

  • Conduct skilled nursing visits to evaluate and execute individualized care plans within patients homes
  • Administer medications, carry out wound care, manage IV therapy, and complete other prescribed clinical tasks
  • Monitor and promptly report patient status changes to the RN Case Manager or physician
  • Educate patients and caregivers on health conditions, medication adherence, and self-care techniques
  • Document all assessments and care provided within Home Care Home Base (HCHB)
  • Ensure thorough care coordination and compliance by collaborating with the interdisciplinary team
  • Maintain high standards of infection control and overall patient safety
  • Participate in regular case conferences, quality improvement initiatives, and mandatory trainings

Qualifications:

  • Active and unrestricted LPN license in the state of practice
  • Minimum of 1 year experience in home health, hospice, or similar field-based environment
  • Proficiency with Home Care Home Base (HCHB) is required
  • Current BLS certification
  • Valid drivers license and reliable transportation with insurance
  • Strong clinical judgment, communication, and time management skills
  • Able to function autonomously and manage a dynamic patient caseload

Strongly preferred candidates bring experience with Medicare-certified agencies and a deep understanding of home health regulatory standards.
