Job Description

A rewarding opportunity is available for a Licensed Practical Nurse (LPN) dedicated to delivering high-quality care in patients' homes. This full-time contract position involves managing a caseload of patients within a defined territory, providing skilled nursing visits tailored to individual care plans. The role prioritizes patient independence, education, and clinical excellence while maintaining effective communication with patients, families, and the healthcare team.

This position is based in a community known for its welcoming atmosphere and scenic surroundings. The area offers a balance of small-town charm and access to city amenities, making it an attractive place to live and work.

Responsibilities:

  • Perform skilled nursing assessments, develop, and implement individualized care plans.
  • Administer medications, wound care, IV therapy, and other clinical treatments as prescribed.
  • Monitor patient conditions and communicate changes promptly to RNs or physicians.
  • Educate patients and caregivers on health management, medication, and self-care.
  • Maintain accurate and timely documentation using Home Care Home Base (HCHB).
  • Collaborate with the interdisciplinary team to ensure coordinated, compliant care.
  • Uphold infection control and patient safety standards.
  • Participate in case conferences, quality improvement initiatives, and ongoing training.

Qualifications and Desired Experience:

  • Active, unrestricted LPN license in the state of practice.
  • Minimum one year of experience in home health, hospice, or community-based care.
  • Proficiency with Home Care Home Base (HCHB) software.
  • Valid driver's license, reliable insured vehicle, and dependable transportation.
  • Current Basic Life Support (BLS) certification.
  • Strong clinical judgment, time management, and communication skills.
  • Ability to work independently in a field-based environment.
  • Preferred: Experience with Medicare-certified home health agencies and home health compliance.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM, with some flexibility as patient needs arise.
  • Approximately 35 patient visits per day, totaling 2532 points weekly.
