Job Description

Deliver compassionate, patient-focused care by joining a respected healthcare team as a Home Health Licensed Practical Nurse (LPN). This full-time contract position is ideal for someone passionate about empowering patients to recover and thrive at home, while supporting their independence and building meaningful relationships.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM
  • Occasional schedule flexibility to accommodate patient needs

Key Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least one year of hands-on experience in home health, hospice, or a similar community care environment
  • Proficiency with Home Care Home Base (HCHB) charting software (required)
  • Valid drivers license, reliable transportation, and current auto insurance
  • Current BLS certification
  • Strong clinical judgment, clear communication, and excellent time management
  • Ability to manage a diverse caseload and function independently in the field

Preferred Experience:

  • Work history with Medicare-certified home health agencies
  • Detailed knowledge of home health compliance and proper documentation
  • Outstanding interpersonal skills and a deeply patient-centered care philosophy

Role Responsibilities:

  • Conduct skilled nursing visits and develop individualized care plans for home-based patients, covering 35 visits daily (2532 points per week)
  • Deliver clinical interventions including medication administration, wound care, and IV therapy
  • Monitor and report changes in patient conditions to the RN Case Manager or physician
  • Provide thorough education on disease management, medications, and self-care to patients and caregivers
  • Maintain meticulous documentation in HCHB according to regulatory and organizational standards
  • Participate in team meetings, case conferences, and ongoing training
  • Adhere to best practices for patient safety and infection control standards
