As a Home Health Licensed Practical Nurse (LPN), youll deliver essential patient care directly in home settings, focusing on fostering independence and positive health outcomes. This contract-based opportunity calls for an experienced nurse able to efficiently manage a caseload, provide skilled interventions, and ensure compliance with all home health regulations.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
• Workload: 2532 points per week (approximately 35 patient visits per day)
• Field-based; travel within assigned territory (reliable vehicle required)

Key Responsibilities:

• Conduct comprehensive nursing assessments, develop individualized care plans, and deliver direct patient care in accordance with physician orders
• Administer medications, manage wound care and IV therapy, and provide other skilled services as needed
• Recognize, document, and report patient condition changes to the RN Case Manager or physician swiftly
• Educate patients and family members on health management, medication protocols, and self-care strategies
• Maintain thorough and accurate patient records in Home Care Home Base (HCHB)
• Collaborate with the healthcare team to coordinate and optimize patient care
• Uphold infection control practices and prioritize patient safety
• Participate in team meetings, training, and quality improvement initiatives

Desired Qualifications:

• Active, unrestricted LPN license
• At least one year of experience in home health, hospice, or community-based care
• Proficiency with Home Care Home Base (HCHB)
• BLS certification and a valid drivers license with reliable, insured transportation
• Sound clinical judgment, effective communication, and organizational skills
• Ability to work autonomously and adapt to changing patient needs

Preferred:

• Experience with Medicare-certified home health agencies
• Robust understanding of compliance standards and documentation requirements

$1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
