Job Description

A dedicated Licensed Practical Nurse is needed to provide compassionate, high-quality care to patients in their homes. This full-time contract position emphasizes patient-centered nursing, helping individuals achieve greater independence and optimal health outcomes. The focus is on strong communication, clinical excellence, and documented compliance with home health regulations.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility required to accommodate patient needs

Key Responsibilities:

  • Deliver skilled nursing visits, assess patient status, and develop individualized care plans
  • Provide medications, wound care, IV therapy, and clinical interventions as ordered
  • Monitor and report changes in patient conditions to the RN Case Manager or physician promptly
  • Educate patients and caregivers on disease management, medication, and self-care practices
  • Ensure accurate, timely documentation of care and assessments using Home Care Home Base (HCHB)
  • Coordinate with interdisciplinary teams to maintain comprehensive, compliant patient care
  • Adhere to infection control and patient safety protocols
  • Take part in case conferences, quality improvement initiatives, and ongoing training

Required Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum of 1 year of experience in home health, hospice, or similar community-based care
  • Proficiency in Home Care Home Base (HCHB) charting system
  • Valid drivers license, reliable transportation, and current automobile insurance
  • BLS (Basic Life Support) certification
  • Strong clinical, organization, and communication skills
  • Ability to independently manage a dynamic field caseload

Preferred Skills:

  • Experience working in Medicare-certified home health
  • Familiarity with home health compliance and documentation standards
  • Excellent interpersonal and patient advocacy abilities
