Job Description
A dedicated Licensed Practical Nurse is needed to provide compassionate, high-quality care to patients in their homes. This full-time contract position emphasizes patient-centered nursing, helping individuals achieve greater independence and optimal health outcomes. The focus is on strong communication, clinical excellence, and documented compliance with home health regulations.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility required to accommodate patient needs
Key Responsibilities:
- Deliver skilled nursing visits, assess patient status, and develop individualized care plans
- Provide medications, wound care, IV therapy, and clinical interventions as ordered
- Monitor and report changes in patient conditions to the RN Case Manager or physician promptly
- Educate patients and caregivers on disease management, medication, and self-care practices
- Ensure accurate, timely documentation of care and assessments using Home Care Home Base (HCHB)
- Coordinate with interdisciplinary teams to maintain comprehensive, compliant patient care
- Adhere to infection control and patient safety protocols
- Take part in case conferences, quality improvement initiatives, and ongoing training
Required Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of 1 year of experience in home health, hospice, or similar community-based care
- Proficiency in Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable transportation, and current automobile insurance
- BLS (Basic Life Support) certification
- Strong clinical, organization, and communication skills
- Ability to independently manage a dynamic field caseload
Preferred Skills:
- Experience working in Medicare-certified home health
- Familiarity with home health compliance and documentation standards
- Excellent interpersonal and patient advocacy abilities
