Schedule/Hours:
- Contract assignment with potential for extension
Key Responsibilities:
- Deliver nursing care in patient homes according to individualized care plans
- Assess, monitor, and document patient condition, updating care strategies as required
- Administer prescribed medications and perform treatments within licensing guidelines
- Provide patient and family education regarding illness management and disease prevention
- Accurately record care activities and patient data using HomeCare HomeBase (HCHB) EMR
- Collaborate with RNs, therapists, and other healthcare professionals to coordinate holistic care
Required Qualifications:
- Current and active LPN license
- At least one year of recent experience in home health settings
- Proficiency with HomeCare HomeBase (HCHB) EMR
- Well-developed clinical assessment and documentation skills
- Reliable transportation for travel to patient homes
- Strong ability to work independently and organize caseload effectively
