Seeking a skilled and compassionate Licensed Practical Nurse (LPN) with a background in home health to provide dedicated patient care in a field-based role. This opportunity focuses on delivering high-quality, patient-centered services directly to individuals in their homes and supporting their recovery and independence.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Occasional flexibility based on patient needs

Key Responsibilities:

• Conduct skilled nursing visits for homebound patients, assessing health needs and implementing individualized care plans
• Administer medications, provide wound care, and manage IV therapies as prescribed
• Monitor and report changes in patient condition to RN Case Manager or physician
• Educate patients and families on disease processes, medication regimens, and self-care practices
• Document all patient contact, interventions, and care activities using Home Care Home Base (HCHB)
• Collaborate with an interdisciplinary team to ensure care plan compliance
• Maintain infection control practices and prioritize patient safety at all times
• Participate in case conferences, training sessions, and quality improvement initiatives

Required Qualifications:

• Current, unrestricted LPN license in the state of practice
• At least 1 year of recent experience in home health or community-based care
• Proficiency in Home Care Home Base (HCHB) for documentation
• Valid drivers license, reliable transportation, and current auto insurance
• Active BLS certification
• Excellent clinical judgment, organizational skills, and communication abilities
• Ability to manage a dynamic caseload independently

Preferred:

• Previous work with Medicare-certified home health agencies
• In-depth knowledge of home health documentation and compliance requirements
• Strong patient advocacy and interpersonal skills
