Step into a vital role where your expertise and compassion directly improve the lives of patients receiving care in their own homes. We are seeking a dedicated Licensed Practical Nurse (LPN) with home health experience and solid clinical skills for a contract opportunity.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Required Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of recent experience in home health, hospice, or similar community-based care
- Proficient with Home Care Home Base (HCHB) electronic medical records
- Valid drivers license, reliable vehicle, and current insurance
- BLS (Basic Life Support) certification
- Strong time management, clinical judgment, and communication abilities
- Ability to work autonomously and manage a dynamic caseload
Preferred Experience:
- Previous work with Medicare-certified home health agencies
- Familiarity with home health compliance and documentation standards
- Excellent interpersonal skills and commitment to patient-centered care
Key Responsibilities:
- Conduct skilled nursing visits in patients homes, following individualized care plans
- Administer medications, perform wound care, and manage clinical interventions as ordered
- Monitor patient conditions, reporting changes promptly to RN Case Manager or physician
- Educate patients and caregivers on diagnoses, medication management, and home self-care
- Thoroughly document care and communication in HCHB
- Coordinate with the interdisciplinary team to ensure comprehensive, compliant care
- Maintain infection control and prioritize patient safety at all times
- Participate in regular case conferences, quality initiatives, and required training
