An opportunity is available for a Licensed Practical Nurse (LPN) specializing in home health care to deliver compassionate, patient-focused nursing services within patients homes. This full-time contract position involves managing a caseload of clients, emphasizing patient education, independence, and recovery, all while maintaining strong communication with the care team and families.

Key Responsibilities:

• Conduct skilled nursing assessments and implement individualized care plans in patients homes.
• Administer medications, provide wound care, and manage IV therapy or other clinical treatments as prescribed.
• Monitor patient condition changes and report promptly to RN Case Manager or physician.
• Educate patients and caregivers regarding disease management, medication, and self-care.
• Accurately document all care and communication using the Home Care Home Base (HCHB) charting system.
• Collaborate with interdisciplinary teams to ensure compliance with care plans and regulatory standards.
• Maintain infection control practices and patient safety during all visits.
• Participate in case conferences, quality improvement initiatives, and ongoing training.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum of 1 year of experience in home health, hospice, or community-based care.
• Proficiency with Home Care Home Base (HCHB).
• Valid drivers license with reliable transportation and current insurance.
• Current Basic Life Support (BLS) certification.
• Strong clinical judgment, time management, and communication skills.
• Ability to work independently in a field-based environment managing a dynamic caseload.

Preferred Experience:

• Background with Medicare-certified home health agencies.
• Knowledge of home health compliance and documentation standards.
• Excellent interpersonal skills with a patient-centered approach.

Schedule and Location:

• Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility as patient needs require.
• Approximately 35 patient visits per day (2532 points weekly).
• Field-based role serving patient homes in the Vail, Arizona area.
• Reliable transportation required; mileage reimbursement typically provided.
