Deliver essential nursing care directly in patients homes as a Licensed Practical Nurse (LPN) specializing in home health services. This role involves managing a caseload of patients within an assigned territory, promoting patient independence, and ensuring compliance with healthcare standards through meticulous documentation and collaboration with a multidisciplinary team.

Key Responsibilities:

• Conduct skilled nursing visits to assess patient conditions, develop and implement individualized care plans.
• Administer medications, perform wound care, and manage IV therapy and other clinical interventions.
• Monitor patient status, report changes to RN Case Managers or physicians promptly.
• Educate patients and caregivers on disease management, medication adherence, and self-care.
• Document all patient interactions and care activities utilizing the Home Care Home Base (HCHB) electronic charting system.
• Collaborate with healthcare professionals to maintain coordinated, compliant patient care.
• Uphold infection control protocols and maintain patient safety.
• Participate in case conferences, quality improvement initiatives, and mandatory training sessions.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum one year of experience in home health, hospice, or community-based care.
• Proficient with Home Care Home Base (HCHB) documentation.
• Valid drivers license, reliable insured vehicle for travel within assigned service areas.
• Current Basic Life Support (BLS) certification.
• Strong clinical judgment, excellent communication, and time management skills.
• Ability to independently manage a dynamic home health caseload.

Preferred Experience:

• Background with Medicare-certified home health agencies.
• Comprehensive knowledge of home health compliance and documentation standards.
• Exceptional interpersonal skills and a patient-focused approach.

Schedule:

• Full-time, Monday through Friday, 8:00 AM to 5:00 PM, with occasional flexibility based on patient needs.
