Job Description

Deliver compassionate, high-quality care in a field-based role serving patients in their homes. This contract opportunity offers experienced Licensed Practical Nurses (LPNs) the chance to build meaningful relationships while supporting patient recovery and independence. Youll leverage your clinical skills and home health expertise to ensure patients receive outstanding, individualized care within the comfort of their residence.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility may be required to meet patient needs

Responsibilities:

  • Provide skilled nursing visits, including assessment, care planning, medication administration, wound care, and IV therapy as needed
  • Monitor and report any changes in condition to the RN Case Manager or physician
  • Educate patients and caregivers on disease management, medication regimens, and self-care strategies
  • Maintain thorough, accurate patient documentation using Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary healthcare teams to ensure seamless, coordinated care
  • Adhere to infection control guidelines and prioritize patient safety in every interaction
  • Participate in case conferences, training, and quality improvement initiatives

Required Qualifications:

  • Active, unrestricted LPN license for the state of practice
  • At least 1 year of home health, hospice, or community-based nursing experience
  • Proficiency with Home Care Home Base (HCHB)
  • Valid drivers license, reliable vehicle, and current auto insurance
  • Current Basic Life Support (BLS) certification
  • Strong clinical judgment, time management, and communication abilities
  • Self-motivation and the ability to manage a varied caseload independently

Preferred Qualifications:

  • Background with Medicare-certified home health agencies
  • In-depth knowledge of home health compliance standards
  • Patient-centered, empathetic approach paired with outstanding interpersonal skills
