Job Description

Deliver expert, compassionate care to patients in the comfort of their homes as a Licensed Practical Nurse supporting a field-based home health team. In this contract role, youll be an essential part of a collaborative interdisciplinary group, focusing on fostering patient independence, providing clinical interventions, and educating patients and families on disease processes and self-care.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility to meet patient care needs

Key Responsibilities:

  • Conduct skilled nursing visits, performing comprehensive assessments and carrying out individualized care plans
  • Administer medications, provide wound care, manage IV therapy, and oversee clinical interventions as ordered
  • Monitor patient conditions, promptly reporting changes to the RN Case Manager or physician
  • Deliver patient and caregiver education regarding medication management, disease processes, and self-care strategies
  • Complete thorough and timely documentation using Home Care Home Base (HCHB)
  • Collaborate closely with the interdisciplinary team, attending case conferences and contributing to ongoing quality improvement
  • Uphold strict infection control and safety standards in all aspects of patient care
  • Stay compliant with all regulatory, organizational, and documentation guidelines

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum 1 year of experience in home health, hospice, or similar community-based care roles
  • Proficient with Home Care Home Base (HCHB) charting system
  • Current BLS certification
  • Valid drivers license, reliable vehicle, and up-to-date insurance
  • Ability to work autonomously, manage a full caseload, and communicate effectively with patients and team members

Preferred Background:

  • Experience in Medicare-certified home health agencies
  • In-depth knowledge of home health compliance standards
  • Outstanding interpersonal and patient advocacy skills
