Provide compassionate and skilled nursing care directly in patients homes as a Licensed Practical Nurse (LPN) specializing in home health. This field-based role requires delivering patient-centered service while managing a dynamic caseload within a supportive interdisciplinary healthcare team. The position offers a full-time contract with a consistent Monday through Friday schedule and the opportunity to make a meaningful impact on patients recovery and independence.

Key Responsibilities
- Perform skilled nursing visits to evaluate patient conditions and develop individualized care plans
- Administer medications, wound care, and therapies such as IV treatments according to medical orders
- Monitor and report changes in patient status to case managers and physicians promptly
- Educate patients and caregivers on disease management, medication, and self-care techniques
- Document all care activities and communications accurately using Home Care Home Base (HCHB) software
- Collaborate with the healthcare team to ensure compliance with care plans and regulatory standards
- Uphold infection control protocols and maintain patient safety in home environments
- Participate in case conferences, quality improvement initiatives, and mandatory training sessions
Qualifications and Experience
- Active, unrestricted LPN license in Oregon
- Minimum of one year of home health or hospice nursing experience
- Proficiency with Home Care Home Base (HCHB) software
- Valid drivers license, reliable transportation, and current auto insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment coupled with effective communication and time-management skills
- Ability to work autonomously in a field setting managing multiple patient visits daily
Preferred Skills
- Experience with Medicare-certified home health agencies
- Deep knowledge of home health regulatory compliance and documentation standards
- Exceptional interpersonal skills and a patient-focused care philosophy
Contract Details and Benefits
- Full-time contract position
- Monday Friday, 8:00 AM to 5:00 PM, with occasional schedule flexibility for patient needs
- Manage approximately 35 patient visits per day (2532 weekly points)
