As a Home Health Licensed Practical Nurse (LPN), you will deliver high-quality clinical services to patients within their own homes, supporting their independence and maximizing health outcomes through education and evidence-based nursing practices. This contract opportunity offers professional autonomy within a structured, collaborative team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
• Workload: 2532 points per week (approximately 35 patient visits per day)
• Field-based in patients homes within an assigned territory

Key Responsibilities:

• Conduct skilled nursing visits to assess, plan, and implement individualized care for home health patients
• Administer medications, wound care, IV therapy, and other clinical interventions per physician orders
• Monitor condition changes, reporting findings promptly to RN Case Manager or physician
• Educate patients and caregivers on disease management, medication administration, and self-care strategies
• Maintain accurate, timely documentation utilizing Home Care Home Base (HCHB)
• Work closely with the interdisciplinary care team to ensure compliance and optimum patient outcomes
• Uphold infection control measures and patient safety practices
• Participate in case conferences, quality initiatives, and required trainings

Qualifications:

• Active and unrestricted LPN license
• At least 1 year of experience in home health, hospice, or related community care setting
• Proficiency with Home Care Home Base (HCHB) is required
• Valid drivers license, reliable transportation, and current insurance
• Current BLS certification
• Clinical judgment, exceptional time management, and effective communication
• Ability to work independently in the field and manage a dynamic caseload

Preferred:

• Experience with Medicare-certified home health agencies
• Strong understanding of compliance and documentation standards
