Advance your nursing career by providing direct, compassionate patient care in a field-based environment. As a Home Health Licensed Practical Nurse (LPN), you will deliver high-quality clinical services to patients within their own homes, supporting their independence and maximizing health outcomes through education and evidence-based nursing practices. This contract opportunity offers professional autonomy within a structured, collaborative team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
- Workload: 2532 points per week (approximately 35 patient visits per day)
- Field-based in patients homes within an assigned territory
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care for home health patients
- Administer medications, wound care, IV therapy, and other clinical interventions per physician orders
- Monitor condition changes, reporting findings promptly to RN Case Manager or physician
- Educate patients and caregivers on disease management, medication administration, and self-care strategies
- Maintain accurate, timely documentation utilizing Home Care Home Base (HCHB)
- Work closely with the interdisciplinary care team to ensure compliance and optimum patient outcomes
- Uphold infection control measures and patient safety practices
- Participate in case conferences, quality initiatives, and required trainings
Qualifications:
- Active and unrestricted LPN license
- At least 1 year of experience in home health, hospice, or related community care setting
- Proficiency with Home Care Home Base (HCHB) is required
- Valid drivers license, reliable transportation, and current insurance
- Current BLS certification
- Clinical judgment, exceptional time management, and effective communication
- Ability to work independently in the field and manage a dynamic caseload
Preferred:
- Experience with Medicare-certified home health agencies
- Strong understanding of compliance and documentation standards
