Job Description
Deliver skilled nursing care as a Home Health Licensed Practical Nurse (LPN) in a rewarding, patient-focused role. This full-time contract position involves providing compassionate, individualized care to patients in their homes, promoting independence, health education, and recovery. You will manage a caseload of approximately 35 patient visits daily and utilize your expertise to support patient wellness in their community setting.
Location: This position is field-based within Stone Mountain, GA, requiring reliable transportation to visit patients within the assigned territory. Mileage reimbursement is typically provided.
Responsibilities include:
- Conducting skilled nursing assessments, planning, and implementing individualized care plans
- Administering medications, wound care, IV therapy, and other clinical treatments
- Monitoring patient conditions and reporting to RN Case Managers or physicians promptly
- Educating patients and caregivers on disease management, medication administration, and self-care
- Documenting all care and communication accurately using Home Care Home Base (HCHB)
- Collaborating with interdisciplinary healthcare teams to ensure care plan compliance
- Maintaining infection control standards and ensuring patient safety at all times
- Participating in case conferences, quality improvement initiatives, and ongoing training
Qualifications:
- Active, unrestricted LPN license in Georgia
- Minimum one year of home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) documentation system
- Valid drivers license and reliable insured vehicle
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication skills, and the ability to work independently in a home environment
Preferred experience includes previous work with Medicare-certified home health agencies and a strong understanding of home health compliance and documentation standards.
