Provide compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) in a dynamic field-based role. This contract position offers the opportunity to support individuals in their homes while promoting independence and recovery within a collaborative healthcare team.
Responsibilities include conducting skilled nursing visits, administering medications and wound care, monitoring patient conditions, and educating patients and caregivers. You will manage a caseload, maintain accurate documentation using Home Care Home Base (HCHB), and ensure compliance with infection control and safety standards. Collaboration with interdisciplinary team members and participation in quality improvement activities are essential.
Qualifications and Desired Experience:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of experience in home health, hospice, or community-based care
- Proficient in Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently in a field-based environment
Preferred Qualifications:
- Experience with Medicare-certified home health agencies
- Knowledge of home health compliance and documentation standards
- Exceptional interpersonal skills with a patient-centered approach
Location:
Based in Stone Mountain, GA, this role involves traveling to patient homes within the assigned territory. Stone Mountain offers a blend of scenic natural beauty and suburban living, with convenient access to metropolitan amenities and a community-focused environment.
Schedule/Hours:
- Monday to Friday, 8:00 AM 5:00 PM (with occasional flexibility for patient needs)
- Approximately 35 patient visits daily (2532 points per week)
