Qualifications:
- Current and unencumbered LPN licensure.
- Solid clinical background, with proven experience in home health.
- Knowledge of home-based care protocols, assessments, and medication administration.
- Ability to work independently and communicate clearly with patients, families, and multidisciplinary teams.
- Professional demeanor, strong problem-solving abilities, and reliable documentation skills.
- Proficiency with electronic charting systems is advantageous.
- Current CPR certification.
Key Responsibilities:
- Deliver professional nursing care to patients in accordance with individualized care plans.
- Conduct home visits, monitor patient progress, and promptly report changes to supervising RNs or providers.
- Administer medications, manage wound or catheter care, and educate patients and family members regarding treatments.
- Accurately document assessments, interventions, and outcomes following regulatory and agency standards.
- Collaborate with physicians, case managers, and caregivers to ensure continuity and quality of care.
- Maintain compliance with agency policies, infection control standards, and all applicable regulations.
