Qualifications:

• Current and unencumbered LPN licensure.
• Solid clinical background, with proven experience in home health.
• Knowledge of home-based care protocols, assessments, and medication administration.
• Ability to work independently and communicate clearly with patients, families, and multidisciplinary teams.
• Professional demeanor, strong problem-solving abilities, and reliable documentation skills.
• Proficiency with electronic charting systems is advantageous.
• Current CPR certification.

Key Responsibilities:

• Deliver professional nursing care to patients in accordance with individualized care plans.
• Conduct home visits, monitor patient progress, and promptly report changes to supervising RNs or providers.
• Administer medications, manage wound or catheter care, and educate patients and family members regarding treatments.
• Accurately document assessments, interventions, and outcomes following regulatory and agency standards.
• Collaborate with physicians, case managers, and caregivers to ensure continuity and quality of care.
• Maintain compliance with agency policies, infection control standards, and all applicable regulations.
