Qualifications:
- Active and unrestricted LPN license
- Previous home health experience preferred
- Proficiency with clinical documentation and electronic health records
- Understanding of care plans and rehabilitative therapies

- Valid drivers license and reliable transportation
Responsibilities:
- Conduct patient assessments and monitor changes in condition
- Administer prescribed medications and treatments
- Follow individualized care plans under the direction of RN or supervisor
- Provide wound care, injections, and monitoring of vital signs
- Educate patients and their families about managing illness or injury at home
- Maintain comprehensive, accurate, and timely patient records
- Communicate effectively with physicians, care coordinators, and other healthcare professionals
- Support continuity of care to promote patient well-being
