Preferred Qualifications
- Current and active LPN license
- Minimum of one year of LPN experience, ideally within home health or related healthcare settings
- Strong assessment, documentation, and communication abilities
- Comfortable working independently in patient home environments
- CPR certification
Responsibilities
- Deliver professional, compassionate nursing care to home health patients under physician orders
- Perform comprehensive patient assessments and report changes in condition to care teams
- Administer prescribed medications and treatments, including wound care and injections
- Provide patient education regarding disease processes, self-care, and medication management
- Maintain accurate and timely clinical documentation
- Work collaboratively with interdisciplinary teams to develop and follow individualized care plans
