An outstanding opportunity is available for a skilled and compassionate Licensed Practical Nurse (LPN) with expertise in home health care. In this field-based contract position, you will deliver high-quality, patient-focused nursing care directly in patients homes, supporting their recovery, independence, and overall well-being. This role requires strong clinical ability, solid understanding of home health compliance, and superior communication with both patients and the multidisciplinary healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility based on patient needs
Key Qualifications:
- Active and unrestricted LPN license in the state of practice
- Minimum of 1 year of recent experience in home health, hospice, or comparable community-based care
- Proficiency with Home Care Home Base (HCHB) required
- Valid drivers license, reliable transportation, and current auto insurance
- Current BLS (Basic Life Support) certification
- Strong time management, clinical judgment, and communication abilities
- Ability to independently manage and prioritize a varied caseload
Preferred Experience:
- Prior work with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation requirements
- Positive, patient-centered attitude and strong interpersonal skills
Role Responsibilities:
- Provide skilled nursing care to home health patients, including assessments, wound care, IV therapy, and medication management
- Conduct 35 patient visits per day (workload of 2532 points per week)
- Deliver comprehensive patient and caregiver education for disease processes and self-care
- Accurately chart patient information and care activities using HCHB
- Maintain compliance with infection control and patient safety standards
- Collaborate with the interdisciplinary healthcare team and participate in ongoing quality initiatives
