Job Description

Provide compassionate, patient-centered care as a Home Health Licensed Practical Nurse (LPN) serving patients in their homes within the Silverton, OR area. This full-time contract position offers the opportunity to deliver skilled nursing services that promote patient independence, education, and recovery while coordinating closely with patients, families, and the interdisciplinary healthcare team.

Schedule & Workload:

  • Monday to Friday, 8:00 AM 5:00 PM with occasional flexibility for patient needs
  • Manage 2532 points per week, equating to approximately 35 patient visits daily
  • Field-based work within an assigned territory requiring reliable transportation

Responsibilities:

  • Perform skilled nursing visits including assessments, care planning, medication administration, wound care, and clinical interventions such as IV therapy
  • Monitor patient conditions and promptly communicate changes to RN case managers or physicians
  • Educate patients and caregivers on disease processes, medication adherence, and self-care skills
  • Document all assessments and care accurately using Home Care Home Base (HCHB) system
  • Collaborate with the healthcare team to ensure compliance with care plans and regulatory standards
  • Uphold infection control protocols and safeguard patient safety throughout service delivery
  • Engage in case conferences, quality improvement, and ongoing training to maintain clinical excellence

Qualifications:

  • Active, unrestricted LPN license in Oregon
  • Minimum of one year experience in home health, hospice, or community-based care
  • Proficiency in Home Care Home Base (HCHB) documentation system
  • Valid drivers license, reliable vehicle, and current insurance
  • Current BLS certification
  • Strong clinical judgment, communication, and time management skills
  • Ability to work independently in a dynamic, field-based setting

Preferred:

  • Experience with Medicare-certified home health agencies
  • In-depth knowledge of home health compliance and documentation standards
  • Exceptional interpersonal skills grounded in patient-focused care
