Job Description
Deliver essential care directly to patients' homes, empowering their independence and recovery as a Home Health Licensed Practical Nurse (LPN). This contract position offers an opportunity to make a significant impact by providing high-quality, patient-focused nursing services within a supportive clinical structure.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility may be needed in response to patient needs
Key Responsibilities:
- Conduct skilled home health nursing visits, assessing and implementing personalized care plans
- Administer medications, perform wound care, and oversee IV therapy and other clinical procedures as authorized
- Evaluate and record changes in patient condition; promptly communicate concerns to RN Case Manager or physician
- Educate patients and caregivers on disease management, medication regimens, and self-care methods
- Maintain precise documentation for all interventions and communications using Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary health team to ensure high standards of compliance and care continuity
- Uphold infection control and safety practices throughout patient care
- Participate in case conferences, quality improvement initiatives, and ongoing training as required
Requirements:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of home health, hospice, or relevant community-based nursing experience
- Proficient with Home Care Home Base (HCHB)
- Valid drivers license, reliable transportation, and current insurance
- Current Basic Life Support (BLS) certification
- Excellent clinical assessment, communication, and time management skills
- Capability to work independently in the field and manage a changing caseload
Preferred Experience:
- Background in Medicare-certified home health agencies
- Thorough knowledge of home health compliance and documentation
