Providing compassionate, high-quality nursing care in patients homes, this role requires a Licensed Practical Nurse (LPN) skilled in home health services and community-based care. The primary focus is to support patient independence and recovery through thorough assessments, clinical interventions, and education, while ensuring compliance with home health standards.

Key Responsibilities:
- Perform skilled nursing visits to assess, plan, and deliver individualized care to home health patients
- Administer medications, provide wound care, and manage IV therapy or other treatments as prescribed
- Monitor patient conditions and communicate changes promptly to RN Case Managers or physicians
- Educate patients and caregivers on managing illness, medications, and self-care practices
- Document all care and patient interactions accurately using Home Care Home Base (HCHB) software
- Collaborate with interdisciplinary teams to coordinate care and maintain regulatory compliance
- Uphold infection control and patient safety standards
- Participate in case conferences, quality improvement efforts, and ongoing training
Qualifications:
- Active, unrestricted LPN license in the state of Arkansas
- Minimum one year experience in home health, hospice, or similar community care
- Proficiency with Home Care Home Base (HCHB) documentation system
- Valid driver's license, reliable vehicle insured for travel with patients
- Current BLS certification
- Strong clinical judgment, organizational ability, and effective communication skills
- Able to work independently managing a full caseload in a field-based setting
Preferred:
- Experience with Medicare-certified home health agencies
- Knowledge of home health compliance and documentation standards
- Exceptional interpersonal skills with a compassionate, patient-focused approach
Schedule & Contract Details:
- Full-time contract position
- Monday through Friday, 8:00 AM 5:00 PM, with occasional flexibility for patient needs
- Approximately 35 patient visits per day (2532 points weekly)
- Mileage reimbursement typically provided
