Job Description

Play a vital role in delivering compassionate and high-quality nursing care in a home health setting. This field-based position focuses on fostering patient independence, promoting recovery, and providing education to patients and their familiesall while collaborating closely with the interdisciplinary healthcare team.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM
  • Occasional flexibility to accommodate patient needs

Responsibilities:

  • Perform skilled nursing visits, assess and implement patient-specific care plans in patients homes
  • Administer medications, perform wound care, and manage IV therapy or other clinical interventions as prescribed
  • Monitor patient conditions and promptly communicate changes to the RN Case Manager or physician
  • Educate patients and caregivers on disease processes, medication administration, and self-care strategies
  • Complete accurate documentation through Home Care Home Base (HCHB)
  • Coordinate care and support with other healthcare professionals to meet patient needs
  • Uphold infection control protocols and ensure the highest standards of patient safety
  • Attend case conferences and participate in quality improvement and training activities

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum of 1 year professional experience in home health, hospice, or community-based care required
  • Proficient with Home Care Home Base (HCHB) electronic charting
  • Valid drivers license and reliable transportation; current insurance required
  • Current Basic Life Support (BLS) certification
  • Excellent clinical judgment, time management, and interpersonal skills
  • Ability to work independently, prioritizing a dynamic caseload

Preferred Experience:

  • Background with Medicare-certified home health agencies
  • Strong grasp of home health compliance and documentation
