Deliver compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) in Shepherdsville, KY. This full-time contract position involves providing skilled nursing services directly in patients homes, focusing on promoting independence, education, and recovery. The role requires managing a caseload of approximately 35 patients daily (2532 points per week), using Home Care Home Base (HCHB) for documentation, while collaborating closely with an interdisciplinary healthcare team.

Responsibilities:

• Conduct skilled nursing visits to assess, plan, and implement individualized care plans.
• Administer medications, wound care, and IV therapy as ordered.
• Monitor patient condition changes and communicate promptly with RN Case Manager or physicians.
• Educate patients and caregivers on disease processes, medication management, and self-care techniques.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Coordinate care with the healthcare team to ensure compliance with care plans.
• Adhere to infection control standards and prioritize patient safety.
• Participate in case conferences, quality improvement, and required training.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum 1 year experience in home health, hospice, or community-based care.
• Proficiency with Home Care Home Base (HCHB).
• Valid drivers license and reliable vehicle with current insurance.
• Current BLS (Basic Life Support) certification.
• Strong clinical judgment, time management, and communication skills.
• Ability to work independently managing a dynamic caseload.

Preferred:

• Experience with Medicare-certified home health agencies.
• In-depth knowledge of home health compliance and documentation.
• Excellent interpersonal skills with a patient-centered care approach.

Job Details:

• Schedule: Monday Friday, 8:00 AM 5:00 PM (flexibility for patient needs).
