Bring your clinical expertise and compassionate care to a meaningful role as a Home Health Licensed Practical Nurse (LPN). In this contract position, youll deliver essential patient-centered nursing care directly in clients homes. Your focus will be on patient independence, education, and recovery, while working collaboratively with families and the wider healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility to accommodate patient needs

Key Responsibilities:

• Conduct skilled nursing visits to assess, plan, and implement tailored care plans
• Deliver medication administration, wound care, IV therapy, and other clinical procedures as ordered
• Monitor and document changes in condition, reporting promptly to the RN Case Manager or physician
• Educate patients and caregivers on medication management, disease processes, and self-care
• Maintain accurate documentation in Home Care Home Base (HCHB) to meet compliance standards
• Collaborate with the interdisciplinary team to coordinate care
• Uphold patient safety and infection control at all times
• Participate in case conferences and ongoing training

Desired Qualifications:

• Active, unrestricted LPN license for the state
• Minimum 1 year of experience in home health, hospice, or a similar setting
• Proficient in Home Care Home Base (HCHB) charting
• Valid drivers license, reliable vehicle, and current insurance
• Current BLS certification
• Excellent clinical judgment, organizational, and communication skills
• Ability to work independently and manage a fluctuating caseload

Preferred:

• Previous work in Medicare-certified home health agencies
• In-depth understanding of home health compliance and documentation

$1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
