Provide compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) delivering skilled services directly in patients' homes within an assigned territory. This role plays a vital part in supporting patient independence, recovery, and education while maintaining strong communication with patients, families, and the interdisciplinary healthcare team.

Key Responsibilities:

• Conduct skilled nursing visits to assess, plan, and implement individualized care plans for home health patients
• Administer medications, perform wound care, and manage IV therapy or other clinical interventions as ordered
• Monitor patients conditions, promptly reporting any changes to the RN Case Manager or physician
• Educate patients and caregivers on disease processes, medication management, and self-care techniques
• Document assessments, care provided, and communications accurately using Home Care Home Base (HCHB)
• Collaborate with the healthcare team to coordinate care and ensure compliance with care plans
• Maintain infection control standards and ensure patient safety throughout all visits
• Participate in case conferences, quality improvement activities, and required training

Qualifications and Desired Experience:

• Active, unrestricted LPN license in the state of practice
• Minimum one year of experience in home health, hospice, or community-based care
• Proficiency in Home Care Home Base (HCHB) charting system
• Valid drivers license, reliable vehicle, and current insurance
• Current BLS (Basic Life Support) certification
• Strong clinical judgment, time management abilities, and clear communication skills
• Ability to work independently managing a dynamic caseload in a field-based setting

Preferred:

• Experience with Medicare-certified home health agencies
• In-depth knowledge of home health regulatory compliance and documentation standards
• Patient-centered approach and excellent interpersonal skills

Schedule & Location:

• Full-time contract, Monday to Friday, 8:00 AM 5:00 PM, with occasional flexibility for patient needs
• Approximately 2532 points weekly, equating to about 35 patient visits per day
• Field-based role serving patients' homes in the Seaford, DE area
• Reliable transportation required; mileage reimbursement typically provided
