Job Description
Provide compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) delivering skilled services directly in patients' homes within an assigned territory. This role plays a vital part in supporting patient independence, recovery, and education while maintaining strong communication with patients, families, and the interdisciplinary healthcare team.
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans for home health patients
- Administer medications, perform wound care, and manage IV therapy or other clinical interventions as ordered
- Monitor patients conditions, promptly reporting any changes to the RN Case Manager or physician
- Educate patients and caregivers on disease processes, medication management, and self-care techniques
- Document assessments, care provided, and communications accurately using Home Care Home Base (HCHB)
- Collaborate with the healthcare team to coordinate care and ensure compliance with care plans
- Maintain infection control standards and ensure patient safety throughout all visits
- Participate in case conferences, quality improvement activities, and required training
Qualifications and Desired Experience:
- Active, unrestricted LPN license in the state of practice
- Minimum one year of experience in home health, hospice, or community-based care
- Proficiency in Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable vehicle, and current insurance
- Current BLS (Basic Life Support) certification
- Strong clinical judgment, time management abilities, and clear communication skills
- Ability to work independently managing a dynamic caseload in a field-based setting
Preferred:
- Experience with Medicare-certified home health agencies
- In-depth knowledge of home health regulatory compliance and documentation standards
- Patient-centered approach and excellent interpersonal skills
Schedule & Location:
- Full-time contract, Monday to Friday, 8:00 AM 5:00 PM, with occasional flexibility for patient needs
- Approximately 2532 points weekly, equating to about 35 patient visits per day
- Field-based role serving patients' homes in the Seaford, DE area
- Reliable transportation required; mileage reimbursement typically provided
