Schedule/Hours:

Key Qualifications:

  • Current, active LPN license
  • At least one year of recent experience in home health care
  • Proficiency with HomeCare HomeBase (HCHB) EMR is required
  • Demonstrated strength in clinical assessment and documentation
  • Reliable transportation to visit patients at their residences
  • Ability to work autonomously and manage a home health caseload

Primary Responsibilities:

  • Deliver skilled nursing care in the patients home in alignment with individualized care plans
  • Monitor patient progress, adjust care plans, and document changes accurately and promptly
  • Administer medications and treatments as directed by state standards and care team guidance
  • Educate patients and families about chronic disease management, medication adherence, and preventive care strategies
  • Maintain comprehensive, timely records within HomeCare HomeBase (HCHB) EMR and ensure regulatory compliance
  • Collaborate seamlessly with RNs, physical therapists, occupational therapists, and other providers for coordinated patient care
