Schedule:
- Contract assignment with potential for extension
Key Responsibilities:
- Deliver skilled nursing care based on individualized patient care plans
- Monitor, assess, and document patient conditions, updating care plans as needed
- Administer prescribed medications and treatments according to state protocols
- Educate patients and families on disease management and preventative care
- Ensure accurate and timely charting using HomeCare HomeBase (HCHB) EMR
- Collaborate closely with registered nurses, therapists, and other healthcare team members
Desired Qualifications:
- Active, unrestricted LPN license
- At least one year of recent experience in a home health setting
- Advanced proficiency in HomeCare HomeBase (HCHB) EMR platform
- Strong clinical assessment skills and attention to documentation accuracy
- Reliable personal transportation to conduct home visits
- Capable of working independently while efficiently managing a caseload
