Deliver compassionate patient care as a Licensed Practical Nurse specializing in home health. This contract role focuses on supporting geriatric patients with high-quality, individualized care in their residences. Youll use your expertise to make a real difference for patients where theyre most comfortabletheir own homes.
Schedule/Hours:
- Contract assignment; potential for extension
Key Qualifications:
- Active LPN license
- At least one year of recent home health experience
- Proficiency with HomeCare HomeBase (HCHB) EMR
- Demonstrated clinical assessment and documentation skills
- Dependable transportation for home visits
- Ability to work independently while managing a patient caseload
- Excellent communication and organizational abilities
Responsibilities:
- Deliver skilled nursing care according to individualized care plans
- Monitor, assess, and document patient conditions and progress
- Update care plans and communicate ongoing needs to the healthcare team
- Administer prescribed medications and perform treatments under state regulations
- Educate patients and families on disease management, medication adherence, and preventive healthcare
- Use HomeCare HomeBase (HCHB) EMR for accurate, timely documentation
- Collaborate closely with RNs, therapists, and other providers to ensure comprehensive care
