Deliver skilled nursing care as a Home Health Licensed Practical Nurse (LPN) in a dynamic field-based role focused on patient-centered services. This contract position offers the opportunity to manage a caseload of patients in their homes within a defined territory, applying your clinical expertise and promoting independence and recovery.

Key Responsibilities:

• Perform skilled nursing visits to assess patient needs and execute individualized care plans.
• Administer medications, wound care, IV therapy, and other clinical procedures as prescribed.
• Monitor patient conditions, promptly communicating changes to the RN Case Manager or physician.
• Educate patients and caregivers on disease management, medication adherence, and self-care techniques.
• Document all care, assessments, and communication accurately using the Home Care Home Base (HCHB) system.
• Collaborate with the interdisciplinary healthcare team to ensure coordinated, compliant patient care.
• Maintain infection control protocols and prioritize patient safety during all visits.
• Engage in case conferences, quality improvement initiatives, and ongoing training activities.

Qualifications:

• Active, unrestricted LPN license within the state.
• Minimum of one year experience in home health, hospice, or community-based care.
• Proficient use of Home Care Home Base (HCHB) charting system.
• Valid drivers license with reliable transportation and insurance.
• Current BLS certification.
• Strong clinical judgment, time management, and communication skills.
• Ability to work independently in a fast-paced, field-based environment.

Preferred Experience:

• Background with Medicare-certified home health agencies.
• Familiarity with home health compliance and regulatory documentation requirements.
• Excellent interpersonal and patient-focused care skills.

Schedule & Location:

• Monday through Friday, 8:00 AM 5:00 PM, with occasional schedule flexibility.
• Expected workload of approximately 2532 points weekly (around 35 patient visits daily).
• Home visits conducted within Salem, Oregon, and surrounding areas.
• Reliable transportation required; mileage reimbursement typically provided.
