Schedule/Hours:
- Contract position; schedule to be discussed during the hiring process.
Key Qualifications:
- Active LPN license
- Minimum of one year of recent experience in home health care
- Proficient in HomeCare HomeBase (HCHB) EMR
- Solid clinical assessment and accurate documentation abilities
- Reliable transportation required for visits to patient homes
- Self-motivated with strong organization and time management skills
Primary Responsibilities:
- Deliver skilled nursing interventions to patients at home, following established care plans
- Monitor, report, and revise care plans in response to patient progress
- Administer medications and treatments in adherence with state regulations
- Provide education to patients and families about care management and disease prevention
- Maintain and update clinical records using HCHB EMR
- Collaborate with RNs, therapists, and other allied health staff to support holistic care
