Job Description

Provide compassionate, patient-centered nursing care in a home health setting as a Licensed Practical Nurse (LPN). This full-time contract role involves managing a caseload of patients within an assigned territory in the Saint Louis, MO area, focusing on promoting patient independence, education, and recovery.

Responsibilities:

• Conduct skilled nursing visits to assess, plan, and implement individualized care plans.
• Administer medications, perform wound care, and manage IV therapy or other clinical treatments.
• Monitor patient condition changes and report promptly to RN Case Manager or physicians.
• Educate patients and caregivers on disease processes, medication management, and self-care.
• Document assessments and care accurately using Home Care Home Base (HCHB).
• Collaborate with the interdisciplinary healthcare team to coordinate care.
• Maintain infection control standards and ensure patient safety.
• Participate in case conferences, quality improvement, and training activities.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum of 1 year experience in home health, hospice, or community-based care.
• Proficiency with Home Care Home Base (HCHB) charting system.
• Valid drivers license, reliable vehicle, and current insurance.
• Current Basic Life Support (BLS) certification.
• Strong clinical judgment, time management, and communication skills.
• Ability to work independently and manage a dynamic caseload.

Preferred:

• Experience with Medicare-certified home health agencies.
• Solid understanding of home health compliance and documentation standards.
• Excellent interpersonal skills and patient-centered care approach.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM (with occasional flexibility).
• Approximately 35 patient visits per day (2532 points weekly).
