Job Description

Make a meaningful impact by providing compassionate, individualized care as a Licensed Practical Nurse (LPN) in a home health setting. This contract position is full-time, field-based, and focuses on serving patients in their homes within a designated territory.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Occasional flexibility to meet patient needs
• Workload: 2532 points per week (approximately 35 patient visits per day)

Key Responsibilities:

• Conduct skilled nursing assessments, implement care plans, and deliver patient-centered interventions in accordance with physicians orders
• Administer medications, manage IV therapy, perform wound care, and provide other clinical tasks as required
• Monitor patient conditions, document changes, and promptly communicate with the RN Case Manager or physician
• Educate patients and caregivers on disease management, medication safety, and self-care strategies
• Complete accurate, timely documentation of all visits and communications using Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary healthcare team to coordinate comprehensive patient care
• Uphold infection control standards and ensure patient safety at every encounter
• Participate in case conferences, quality improvement initiatives, and ongoing training sessions

Qualifications:

• Active, unrestricted LPN license in the state of practice
• At least 1 year of recent experience in home health, hospice, or a similar community setting
• Proficiency with Home Care Home Base (HCHB) required
• Valid drivers license and access to a reliable, insured vehicle
• Current Basic Life Support (BLS) certification
• Excellent clinical judgment, organization, and interpersonal communication skills
• Ability to work independently and manage a dynamic caseload

Preferred Experience:

• Familiarity with Medicare-certified home health agencies
• Strong knowledge of home health compliance and documentation standards
