Job Description

Delivering compassionate nursing care in the comfort of patients homes, this position offers a rewarding opportunity for an experienced Home Health Licensed Practical Nurse (LPN) to make a meaningful impact. As a field-based nurse, you will be trusted to promote patient recovery and independence by providing high-quality care and education, while maintaining strong communication with patients, families, and an interdisciplinary healthcare team.

Schedule:

  • Monday through Friday, 8:00 AM to 5:00 PM
  • Occasional flexibility required to accommodate patient needs

Key Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum one year of experience in home health, hospice, or similar community-based setting
  • Proficiency using Home Care Home Base (HCHB) for charting and documentation
  • Valid drivers license, reliable transportation, and current insurance
  • Current BLS (Basic Life Support) certification
  • Ability to work independently and manage a dynamic caseload of 2532 points weekly
  • Excellent clinical judgment, time management, and communication skills

Additional Preferred Experience:

  • Background with Medicare-certified home health agencies
  • Solid understanding of home health compliance and documentation standards
  • Interpersonal strengths and a patient-focused approach

Role Responsibilities:

  • Conduct skilled home visits, assess patient needs, and implement individualized care plans
  • Administer medications, wound care, IV therapy, and other clinical interventions
  • Monitor and report changes in patient condition to RN Case Manager or physician
  • Educate patients and caregivers on self-care and medication management
  • Accurately document care, communication, and assessments using HCHB
  • Collaborate closely with the interdisciplinary team for coordinated, compliant care
  • Maintain rigorous infection control and prioritize patient safety
  • Participate in case conferences, quality initiatives, and required training
