Job Description
This opportunity calls for an experienced and compassionate Licensed Practical Nurse to provide critical home health care services within a field-based setting. You will be the primary point of contact for patients recovering at home, utilizing your clinical skills to deliver high-quality, patient-centered care, promote independence, and offer vital education to individuals and their families.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility required based on patient needs
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of home health, hospice, or similar community-based experience
- Solid proficiency with the Home Care Home Base (HCHB) charting system
- Current BLS (Basic Life Support) certification
- Valid drivers license, reliable transportation, and current auto insurance
- Excellent clinical judgment, independent work capability, strong communication, and organizational skills
Preferred Experience:
- Background with Medicare-certified home health agencies
- Deep knowledge of home health compliance and documentation regulations
- Exceptional interpersonal skills and a demonstrable commitment to patient-centered care
Role Responsibilities:
- Conduct skilled nursing visits (approximately 35 patients per day) and develop individualized care plans
- Perform medication administration, wound care, IV therapy, and other clinical duties as ordered
- Monitor and document patient status using Home Care Home Base (HCHB)
- Educate patients and caregivers on disease management and self-care
- Coordinate with the interdisciplinary team and maintain care plan compliance
- Uphold infection control standards and ensure ongoing patient safety
- Attend case conferences, participate in quality initiatives, and complete necessary training
