Job Description
Experienced Home Health Licensed Practical Nurse (LPN) needed in Sahuarita, AZ, to deliver compassionate, patient-focused nursing care within home settings. This full-time contract role requires a Licensed Practical Nurse dedicated to enhancing patient independence, education, and recovery through skilled clinical interventions and effective communication with patients, families, and healthcare teams.
Responsibilities:
- Perform skilled nursing visits to assess, plan, and implement individualized care plans for home health patients
- Administer medications, conduct wound care, and manage IV therapy or other clinical treatments
- Monitor patient conditions and promptly communicate changes to RN Case Managers or physicians
- Educate patients and caregivers on disease management, medication use, and self-care techniques
- Document all patient assessments, care provided, and communications using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary teams to ensure care coordination and regulatory compliance
- Maintain infection control standards and prioritize patient safety
- Participate in case conferences, quality improvement initiatives, and training sessions
Qualifications:
- Active, unrestricted LPN license in the practicing state
- Minimum one year of experience in home health, hospice, or community-based care
- Proficiency with Home Care Home Base (HCHB) electronic charting
- Valid drivers license, reliable insured vehicle, and mileage reimbursement eligibility
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, effective time management, and communication skills
- Ability to work independently managing a dynamic caseload
Preferred:
- Experience with Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation standards
- Strong interpersonal skills with a patient-centered approach to care
