Job Description
Provide compassionate, patient-centered nursing care as a Licensed Practical Nurse (LPN) in home health, delivering skilled visits to individuals in their homes within the Roseburg, OR area. This full-time, contract position involves managing a caseload of approximately 35 patient visits daily (2532 points per week) Monday through Friday, 8:00 AM 5:00 PM, with occasional schedule flexibility to meet patient needs. Reliable transportation is required for travel within the assigned territory, with typical mileage reimbursement.
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans
- Administer medications, provide wound care, and manage IV therapy or other ordered clinical interventions
- Monitor patient conditions, promptly reporting changes to RN Case Managers or physicians
- Educate patients and caregivers on disease management, medication administration, and self-care strategies
- Accurately document assessments and care in the Home Care Home Base (HCHB) system
- Collaborate with interdisciplinary healthcare team members to coordinate care and ensure compliance with regulatory standards
- Maintain infection control and patient safety protocols
- Participate in case conferences, quality improvement, and required training
Qualifications:
- Active, unrestricted LPN license in Oregon
- Minimum one year of home health, hospice, or community-based care experience
- Proficiency in Home Care Home Base (HCHB) electronic charting
- Valid drivers license, reliable insured vehicle
- Current BLS certification
- Strong clinical judgment, time management, and communication skills
- Ability to work independently in the field managing a dynamic patient caseload
Preferred:
- Experience with Medicare-certified home health agencies
- Comprehensive knowledge of home health compliance and documentation standards
- Exceptional interpersonal skills and patient-focused care approach
