Job Description

We are searching for an experienced and compassionate Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in a home health setting. As a dedicated field nurse, you will help clients achieve greater independence, facilitate recovery, and ensure comprehensive support in the comfort of their homes. Youll manage a rewarding caseload, collaborate closely with the interdisciplinary healthcare team, and apply your home health expertise to make a meaningful impact each day.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility to accommodate patient needs

Responsibilities:

• Deliver skilled nursing visits, assessing and implementing individualized care plans for home health patients
• Administer medications, wound care, and IV therapy as ordered
• Monitor changes in patient condition and communicate findings to RN Case Manager or physician
• Educate patients and caregivers on disease management, medication administration, and self-care routines
• Complete timely and accurate documentation in Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary team to coordinate and ensure comprehensive care
• Adhere to infection control and patient safety guidelines at all times
• Participate in case conferences, quality improvement initiatives, and ongoing training

Qualifications:

• Active, unrestricted LPN license in the state of practice
• Minimum of 1 year experience in home health, hospice, or a similar care setting
• Proficiency in Home Care Home Base (HCHB)
• Valid drivers license, current auto insurance, and reliable transportation
• Current BLS certification
• Strong clinical judgment, organizational, and communication skills
• Ability to work independently and manage a dynamic caseload
• Preferred: Prior Medicare-certified home health experience and knowledge of compliance standards

$1,423 - $1,505/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
