Deliver essential nursing care and make a significant impact by providing home health services to patients in a field-based, home setting. In this contract opportunity, youll play a pivotal role, promoting patient independence, education, and well-being while collaborating closely with families and healthcare professionals.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Occasional flexibility required based on patient needs

Key Responsibilities:

• Conduct skilled nursing visits to assess patient conditions and implement personalized care plans
• Administer medications, wound care, and manage IV therapies or other clinical interventions as ordered
• Monitor patient status and promptly report any changes to the RN Case Manager or physician
• Guide patients and caregivers on medication management, self-care, and disease processes
• Accurately document all assessments, interventions, and communications using Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary team to support a coordinated approach to care
• Uphold infection control practices and maintain a safe environment
• Engage in case conferences, participate in ongoing training, and contribute to quality improvement efforts

Qualifications:

• Active, unrestricted LPN license in Georgia
• Minimum of one years experience in home health, hospice, or related community care
• Solid proficiency in Home Care Home Base (HCHB)
• Current BLS certification
• Valid drivers license, reliable insured vehicle, and comfort with field-based travel
• Effective time management, documentation, and communication abilities
• Ability to work both autonomously and as part of a care team

Preferred:

• Exposure to Medicare-certified home health agencies
• Deep knowledge of home health compliance and documentation
• Strong relationship-building and patient-centered care skills

$1,423 - $1,505/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
