Job Description

Seeking an experienced and dedicated Licensed Practical Nurse (LPN) to deliver compassionate home health care for patients within their own residences. In this field-based role, youll provide direct nursing care, foster patient independence, and maintain strong communication with patients, families, and fellow healthcare professionals. This contract position focuses on supporting recovery through individualized care while conforming to the highest clinical and regulatory standards.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • Occasional flexibility based on patient needs

Key Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least 1 year of experience in home health, hospice, or a comparable community-based setting
  • Proficiency with Home Care Home Base (HCHB) required
  • Valid drivers license, reliable transportation, and current auto insurance
  • Current BLS (Basic Life Support) certification
  • Excellent time management, clinical judgment, and communication skills
  • Comfortable working independently and managing a variable caseload

Preferred Experience:

  • Previous employment with Medicare-certified home health agencies
  • Strong working knowledge of home health compliance standards
  • Outstanding interpersonal skills and a patient-focused demeanor

Primary Responsibilities:

  • Conduct skilled nursing visits and develop individualized care plans
  • Administer medications, perform wound care, and oversee IV therapy as needed
  • Monitor patient status and report significant changes promptly to RN Case Manager or physician
  • Educate patients and caregivers on disease management and safe self-care practices
  • Ensure timely, accurate documentation using HCHB
  • Collaborate with interdisciplinary team members and participate in case conferences and quality activities
  • Uphold infection control procedures and prioritize patient safety at all times
