Job Description
Step into a rewarding opportunity providing essential nursing care to individuals in their homes as a dedicated Home Health Licensed Practical Nurse (LPN). This full-time contract role is ideal for an experienced and compassionate nurse with a background in home health, offering a consistent Monday through Friday schedule from 8:00 AM to 5:00 PM, with some flexibility to accommodate patient needs. Serving a varied caseload of 2532 points per week, youll complete around 35 patient visits daily.
Key Responsibilities:
- Conduct skilled nursing visits to develop and implement individualized care plans for patients at home.
- Administer medications, perform wound care, and manage IV therapy or clinical interventions as ordered.
- Monitor and assess changes in patient condition, promptly communicating findings to the RN Case Manager or physician.
- Educate patients and caregivers on disease processes, medications, and self-care strategies.
- Maintain thorough documentation of care using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary healthcare team members to coordinate care and ensure plan compliance.
- Uphold rigorous infection control and patient safety protocols.
- Participate in case conferences, quality improvement initiatives, and ongoing training.
Required Qualifications:
- Active, unrestricted LPN license for the state of practice.
- At least 1 year of home health, hospice, or related community-based care experience.
- Proficiency with Home Care Home Base (HCHB) charting system.
- Valid drivers license, reliable vehicle, and current insurance.
- Current BLS (Basic Life Support) certification.
- Exceptional clinical judgment, independence, and communication skills.
Preferred Experience:
- Background with Medicare-certified home health agencies.
- In-depth knowledge of home health documentation and compliance standards.
- Patient-centered communication and strong interpersonal skills.
