Job Description

Step into a rewarding opportunity providing essential nursing care to individuals in their homes as a dedicated Home Health Licensed Practical Nurse (LPN). This full-time contract role is ideal for an experienced and compassionate nurse with a background in home health, offering a consistent Monday through Friday schedule from 8:00 AM to 5:00 PM, with some flexibility to accommodate patient needs. Serving a varied caseload of 2532 points per week, youll complete around 35 patient visits daily.

Key Responsibilities:

  • Conduct skilled nursing visits to develop and implement individualized care plans for patients at home.
  • Administer medications, perform wound care, and manage IV therapy or clinical interventions as ordered.
  • Monitor and assess changes in patient condition, promptly communicating findings to the RN Case Manager or physician.
  • Educate patients and caregivers on disease processes, medications, and self-care strategies.
  • Maintain thorough documentation of care using Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary healthcare team members to coordinate care and ensure plan compliance.
  • Uphold rigorous infection control and patient safety protocols.
  • Participate in case conferences, quality improvement initiatives, and ongoing training.

Required Qualifications:

  • Active, unrestricted LPN license for the state of practice.
  • At least 1 year of home health, hospice, or related community-based care experience.
  • Proficiency with Home Care Home Base (HCHB) charting system.
  • Valid drivers license, reliable vehicle, and current insurance.
  • Current BLS (Basic Life Support) certification.
  • Exceptional clinical judgment, independence, and communication skills.

Preferred Experience:

  • Background with Medicare-certified home health agencies.
  • In-depth knowledge of home health documentation and compliance standards.
  • Patient-centered communication and strong interpersonal skills.
