Providing patient-centered nursing care in home settings, this opportunity is ideal for an experienced Licensed Practical Nurse (LPN) skilled in home health services. You will manage a caseload of patients, ensuring that care plans and documentation meet regulatory standards while promoting patient independence and education.

Role Type:
Contract, full-time
Schedule:
- Monday to Friday
- 8:00 AM 5:00 PM (flexibility may be needed for patient appointments)
Key Responsibilities:
- Conduct skilled nursing visits assessing and implementing individualized care plans in patients' homes
- Administer medications, perform wound care, and manage clinical treatments as ordered
- Monitor patient conditions and communicate promptly with RN Case Manager or physicians
- Educate patients and caregivers on disease management, medication, and self-care
- Document all care activities and communication accurately using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary healthcare teams to coordinate and comply with care plans
- Uphold infection control practices and ensure patient safety
- Participate in case conferences, quality improvement initiatives, and mandatory training
Qualifications & Desired Experience:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year home health, hospice, or similar community care experience
- Proficiency with Home Care Home Base (HCHB) software
- Valid drivers license, reliable insured vehicle
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, time management, and communication skills
- Ability to independently manage a dynamic caseload in a field-based setting
Preferred:
- Experience with Medicare-certified home health agencies
- Solid understanding of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-focused approach
