Job Description

Providing patient-centered nursing care in home settings, this opportunity is ideal for an experienced Licensed Practical Nurse (LPN) skilled in home health services. You will manage a caseload of patients, ensuring that care plans and documentation meet regulatory standards while promoting patient independence and education.

Location:

Role Type:
Contract, full-time

Schedule:

  • Monday to Friday
  • 8:00 AM 5:00 PM (flexibility may be needed for patient appointments)

Key Responsibilities:

  • Conduct skilled nursing visits assessing and implementing individualized care plans in patients' homes
  • Administer medications, perform wound care, and manage clinical treatments as ordered
  • Monitor patient conditions and communicate promptly with RN Case Manager or physicians
  • Educate patients and caregivers on disease management, medication, and self-care
  • Document all care activities and communication accurately using Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary healthcare teams to coordinate and comply with care plans
  • Uphold infection control practices and ensure patient safety
  • Participate in case conferences, quality improvement initiatives, and mandatory training

Qualifications & Desired Experience:

  • Active, unrestricted LPN license in the state of practice
  • Minimum 1 year home health, hospice, or similar community care experience
  • Proficiency with Home Care Home Base (HCHB) software
  • Valid drivers license, reliable insured vehicle
  • Current Basic Life Support (BLS) certification
  • Strong clinical judgment, time management, and communication skills
  • Ability to independently manage a dynamic caseload in a field-based setting

Preferred:

  • Experience with Medicare-certified home health agencies
  • Solid understanding of home health compliance and documentation standards
  • Excellent interpersonal skills with a patient-focused approach
