Deliver essential care and make a genuine impact as an experienced Home Health Licensed Practical Nurse (LPN), serving patients within their homes. This position provides the opportunity to support individuals on their path to health and recovery, empowering patients and families while maintaining standards of clinical excellence.
Schedule:
- Full-time contract, Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
- Field-based position (home visits within assigned territory)
- Expect 2532 points per week (approximately 35 patient visits daily)
Your Responsibilities:
- Perform hands-on nursing visits, including assessment, plan of care development, medication administration, wound care, and IV therapy
- Monitor patient progress, noting changes and reporting findings to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medications, and self-care strategies
- Ensure all clinical documentation is completed in Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary care team and attend case conferences
- Practice infection control and ensure safety in patient environments
- Participate in ongoing training and quality improvement activities
Your Qualifications:
- Active and unrestricted LPN license in the state of practice
- Minimum 1 year of home health, hospice, or relevant community-based care experience
- Strong proficiency in Home Care Home Base (HCHB)
- Current BLS (Basic Life Support) certification
- Valid drivers license, reliable vehicle, and current insurance
- Excellent organizational, time management, and communication skills
- Ability to manage an independent, field-based caseload
- Preferred: Experience with Medicare-certified home health agencies and strong home health compliance/documentation skills
