Job Description

Embark on a rewarding opportunity delivering compassionate, personalized care as a Licensed Practical Nurse in a home health setting. In this field-based contract position, you'll play a pivotal role in supporting patients' independence and recovery, managing a defined caseload as you travel between homes within your assigned area.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility for patient needs

Key Responsibilities:

  • Conduct skilled nursing visits to assess patient status and implement individualized care plans
  • Administer prescribed medications, perform wound care, and manage IV therapy as ordered
  • Monitor patient conditions; promptly report significant findings to the RN Case Manager or physician
  • Provide education to patients and caregivers on disease processes, medication usage, and self-care
  • Accurately document assessments, care provided, and interdisciplinary communications using Home Care Home Base (HCHB)
  • Collaborate with team members to ensure coordinated, compliant delivery of care
  • Adhere to infection control protocols to maintain patient safety
  • Participate in case conferences and ongoing trainings

Requirements:

  • Active, unrestricted LPN license in the state of practice
  • Minimum one year of professional experience in home health, hospice, or community-based care
  • Proficiency in Home Care Home Base (HCHB) required
  • Valid drivers license, insured reliable vehicle, and willingness to travel within the assigned territory
  • BLS (Basic Life Support) certification
  • Excellent clinical judgment, organizational, and communication skills
  • Ability to work independently and manage a dynamic caseload

Preferred:

  • Prior work with Medicare-certified home health agencies
  • In-depth knowledge of home health compliance and documentation
