Bring your expertise and compassion as a Home Health Licensed Practical Nurse (LPN) providing critical care in the community. In this rewarding contract position, youll deliver patient-centered nursing services to individuals in their homes, helping to foster patient independence and improve quality of life through education and hands-on care. Strong communication and collaboration with patients, families, and the wider healthcare team are key components of this opportunity.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility required based on patient needs
Key Responsibilities:
- Conduct skilled nursing visits to assess and implement individualized care plans for home health patients
- Administer medications, provide wound care, and perform IV therapy or other clinical interventions as ordered
- Monitor patient conditions, reporting findings to the RN Case Manager or physician promptly
- Offer patient and caregiver education on disease management, medication administration, and self-care strategies
- Document all assessments, care provided, and communication in Home Care Home Base (HCHB)
- Collaborate with an interdisciplinary team to coordinate care and uphold compliance with care plans
- Adhere to infection control standards and ensure safety during all interactions
- Participate in team meetings, case conferences, and required training/quality initiatives
Requirements:
- Active, unrestricted LPN license for the state of practice
- At least 1 year of home health, hospice, or similar experience
- Proficiency in Home Care Home Base (HCHB) charting
- Valid drivers license, insured and reliable vehicle
- Current BLS certification
- Outstanding clinical judgment, communication, and time management
- Ability to work autonomously in diverse home environments
Preferred:
- Past work with Medicare-certified home health agencies
- Deep understanding of home health compliance standards
- Strong interpersonal skills and a commitment to patient-focused care
